This is based around the concept in Lovecraft's writings that the human mind has the capacity of sanity, or to hold the truths of the univers, but not both. The most unique feature of Call of Cthulhu is that each character has a Sanity rating, and throughout the game the horrifying things that occur will lower the character's sanity, possibly causing mental disorders. The game uses very simple rules (a percentage system) and focuses instead on the roleplaying aspects. Lovecraft and other authors whose combined works are referred to as the Cthulhu Mythos.
